How To Claim Your 14 Day Trial Membership Pass
Step 1
Complete the Trial Membership Coupon. If you don't have a coupon,
refer to the instructions for use with the coupon submission form instead.
Step 2
Complete the on-line application.
Step 3
Submit both the application and coupon to the Membership Office at 2311 Campus Dr, Evanston along with a valid photo ID.* Check here for office hours.
* Although you may complete the application form on-line, we are required to obtain an original signature. Therefore, your application must be provided in person to the membership office at 2311 Campus Dr. If you need further assistance, please contact the Membership office at 847 491-4303.
